Plavix (clopidogrel)

Indications

Plavix is used to:

  • Treat acute coronary syndrome.
  • Prevent new heart attacks or strokes in people who have had a recent heart attack or stroke.
  • Treat peripheral arterial disease.
  • Plavix has been shown to reduce the rate of heart attacks and strokes.

Dosage and Administration

  • Read the Medication Guide provided by your pharmacist before starting Plavix and each time you get a refill. If you have any questions, ask your healthcare provider.
  • Take this tablet by mouth, with or without food, as directed by your healthcare provider, usually once daily.
  • Take this medication regularly to get the most benefit. Take it at the same time each day to help you remember.
  • Follow your healthcare provider’s instructions if you are taking Plavix to prevent blood clots after a stent implant or other procedure. This medication may need to be taken with aspirin for several months to years, depending on the procedure and type of stent.
  • Continue taking this tablet even if you feel well.
  • Seek medical help immediately if you experience signs of a new heart attack or stroke, such as chest pain, shortness of breath, unusual sweating, weakness on one side of the body, trouble speaking, or sudden vision changes.
  • Avoid eating grapefruit or drinking grapefruit juice while taking Plavix, as it can increase the risk of side effects.
  • Do not stop taking Plavix without consulting your healthcare provider, as this can increase the risk of cardiovascular events.

Mechanism of Action

Clopidogrel, the active ingredient in Plavix, is an antiplatelet medication. It works by inhibiting platelets from sticking together and forming blood clots. This helps to maintain blood flow and prevent heart attacks and strokes.

Contraindications

Plavix should not be used if you have:

  • Active pathological bleeding, such as a peptic ulcer or intracranial hemorrhage.
  • Hypersensitivity to Clopidogrel or any component of the product.

  • Stomach pain
  • Indigestion

Serious Side Effects:

Precautions

  • CYP2C19 inhibitors: Avoid using omeprazole or esomeprazole with Plavix as they can interfere with the effectiveness of the medication.
  • Bleeding: Plavix increases the risk of bleeding. Monitor for signs of bleeding and report any unusual bleeding to your healthcare provider.
  • Discontinuation: Do not stop taking Plavix prematurely without consulting your healthcare provider, as it can increase the risk of cardiovascular events. If you need elective surgery, stop Plavix 5 days prior to the procedure to reduce the risk of bleeding.
  • Thrombotic thrombocytopenic purpura (TTP): This rare but serious condition has been reported. Seek immediate medical attention if you experience symptoms such as easy bruising, red or purple spots on the skin, or unusual weakness.
  • Cross-reactivity among thienopyridines: If you have had an allergic reaction to another thienopyridine drug (such as ticlopidine), use Plavix with caution.
