Eliquis (apixaban)

Indications and Use

Eliquis is a factor Xa inhibitor indicated for:

  • Reducing the risk of stroke and systemic embolism in patients with nonvalvular atrial fibrillation.
  • Prophylaxis of deep vein thrombosis (DVT), which may lead to pulmonary embolism (PE), in patients who have undergone hip or knee replacement surgery, and
  • Treatment of DVT and PE, and for the reduction in the risk of recurrent DVT and PE following initial therapy.

Dosage and Administration

  • Take Eliquis by mouth with or without food as directed by your healthcare provider. The typical dosing regimen for most patients is usually twice daily, approximately every 12 hours.
  • Follow the duration of treatment based on the type of surgery, if prescribed for prevention after surgery.
  • Take regularly at the same times each day.
  • Consult your healthcare provider before crushing and mixing with liquids if having trouble swallowing the tablet whole.
  • Do not stop taking Eliquis without consulting your healthcare provider, as some conditions may worsen if the medication is suddenly stopped.

Ingredients of Apixaban

Active ingredient: apixaban

Inactive ingredients: anhydrous lactose, microcrystalline cellulose, croscarmellose sodium, sodium lauryl sulfate, and magnesium stearate.

Mechanism of Action

Eliquis (apixaban) inhibits factor Xa, disrupting thrombin generation and fibrin clot formation. This specific action reduces the risk of stroke, systemic embolism, and thromboembolic events associated with atrial fibrillation, DVT, and PE.

Contraindications

  • Active pathological bleeding
  • Severe hypersensitivity to Eliquis

Side Effects of Apixaban

Less serious side effects:

  • Minor bleeding: nosebleeds, light bruising, bleeding from the gums
  • Inform healthcare provider if signs of minor bleeding occur or if blood is noticed in urine or stool.

Serious side effects:

  • Bleeding
  • Spinal blood clots with spinal procedures
  • Increased risk of blood clots if treatment is stopped early
  • Severe allergic reactions

Precautions

  • Premature discontinuation of Eliquis increases the risk of thrombotic events. If Eliquis is stopped for reasons other than pathological bleeding or completion of therapy, you should consider switching to another anticoagulant for continued coverage.
  • Eliquis elevates the risk of bleeding, potentially leading to serious or fatal outcomes. Concurrent use of medications affecting hemostasis, such as aspirin, other antiplatelet agents, anticoagulants, heparin, thrombolytic agents, SSRIs, SNRIs, and NSAIDs, can further heighten the bleeding risk.
  • Use of Eliquis carries a risk of spinal/epidural hematoma, especially in the context of neuraxial intervention. So, carefully weigh the benefits and risks before proceeding with neuraxial interventions in patients already anticoagulated or slated for anticoagulation.
  • Halt the medication for at least 48 hours before elective surgery or invasive procedures associated with a moderate or high risk of unacceptable or clinically significant bleeding. For surgeries presenting a low risk of bleeding, discontinue at least 24 hours before the procedure.

Storage Instructions

Store at 20°C to 25°C (68°F-77°F); excursions permitted between 15°C and 30°C (59°F-86°F).
