What is Trelegy Ellipta? 

Trelegy Ellipta is a brand-name medication that can only be obtained with a doctor’s prescription. This is used in adults with chronic obstructive pulmonary disease (COPD), including emphysema and/or bronchitis, to relieve airflow obstruction. This is also to reduce flare-ups in patients with a history of flare-ups (exacerbations).

Trelegy Ellipta is a combination of three active medications: fluticasone furoate, umeclidinium, and vilanterol.

Fluticasone furoate is an inhaled corticosteroid (ICS) that works by attaching to receptors in various types of immune cells and then decreasing the activity of the immune system. This results in reducing the release of the substances involved in the inflammation process such as histamine, thereby decreasing inflammation of the airways and opening them to help breathing become easier.

Umeclidinium is a long-acting muscarinic antagonist (LAMA) or often referred to as an anticholinergic. It works by attaching to muscarinic receptors and blocking their activity. This drug has a high affinity for the M3 receptor found on the bronchial muscles and is responsible for bronchi constriction. The binding of umeclidinium to M3 results in relaxation of the muscles of the airways, leading to easier breathing.

Vilanterol is a long-acting beta-2 adrenergic agonist (LABA) that works by attaching to and activating beta-2 receptors in the lungs. The activation of the beta-2 receptors leads to the adenylate cyclase enzyme in the bronchial smooth muscles which converts adenosine triphosphate (ATP) to cyclic adenosine monophosphate (cAMP). Higher cAMP results in relaxation of the airways, helping the airways to stay open and making breathing easier.

Trelegy Ellipta dosage

Trelegy ellipta is an inhalation powder containing 100 or 200 mcg fluticasone furoate per blister. The other strip includes a blend of 62.5 mcg umeclidinium and 25 mcg vilanterol per blister. 

The recommended dose for adults with COPD as maintenance treatment is one inhalation (puff) of Trelegy Ellipta 100 mcg/62.5 mcg/25 mcg orally once daily.  

For maintenance treatment of asthma, the recommended initial dosage of Trelegy Ellipta is one inhalation/puff which delivers 100 mcg fluticasone, 62.5 mcg umeclidinium and 25 mcg vilanterol, or 200 mcg fluticasone, 62.5 mcg umeclidinium, and 25 mcg vilanterol once a day, by oral inhalation. 

The maximum dosage is one inhalation/puff of Trelegy Ellipta 200/62.5/25 mcg once daily.  

Directions 

Use Trelegy Ellipta inhaler exactly as prescribed by your healthcare provider. Read carefully and follow the directions on the prescription label and instructions for use. 

When using your inhaler, open the cover first to see the mouthpiece. There’s no need for you to shake this inhaler. Trelegy Ellipta’s inhaler is now ready to be used. 

Hold the inhaler away from the mouth and breathe fully (exhale). 

Place the mouthpiece between your lips and make a good seal by closing your lips around it.

Take a long, steady, deep breath through your mouth, not through your nose. Do not block the air vent with your fingers. 

Remove the inhaler from your mouth and hold your breath for 3-4 seconds or for as long as you can comfortably do so. 

You may not taste or feel the medicine even if you used it correctly. If you cannot taste or feel the medication after using it, do not take an extra dose.

You may clean the mouthpiece using dry tissue before closing the cover. 

Rinse your mouth with water after using the inhaler and spit out the water, do not swallow it.

Cautions

Do not use Trelegy Ellipta if you are allergic to its active and inactive components or milk proteins. 

Do not use Trelegy Ellipta if you experience sudden breathing problems. Be sure to always have your rescue inhaler with you to use to treat sudden symptoms. 

Do not use Trelegy Ellipta in children under 18 years old because it is unknown if the Trelegy Ellipta inhaler is effective and safe for them.

Do not increase your dose or use this medicine more often than prescribed.

Trelegy Ellipta has vilanterol, a long-acting beta2-adrenergic agonist (LABA). When you use Laba medicines such as vilanterol alone, your risk for hospitalization or death from asthma problems will be increased.

Do not take this medicine with other medicines containing LABA or anticholinergic.

Contact your doctor or seek medical help immediately if your breathing problems become worse, if you need your rescue inhaler more frequently than usual, or if it is ineffective to relieve your symptoms.
